HIP REHABILITATION PROTOCOLS

Precision-Engineered Recovery Pathways by SIGMA Orthopedics & Sports Medicine

At SIGMA Orthopedics, your rehabilitation is not an afterthought — it’s a mission-critical part of your hip recovery.

Hip Rehab Done Right: The SIGMA 100-Day Recovery Protocol

Recovering from hip surgery or a serious hip injury isn’t just about “doing exercises” — it’s about structured phases, surgeon-led benchmarks, and seamless integration between your therapy team and your surgeon. At SIGMA Orthopedics, our hip PT protocols were developed by Dr McCormick and reflect his precision approach to hip preservation, arthroscopy, tendon repair and replacement. These protocols guide you — and your physical therapist — through swelling control, motion restoration, strength rebuilding, functional progression, and return to sport or lifestyle. Downloadable, phase-based, and designed for clarity, they keep everyone on the same page from day one.

Watch this video to get a clear overview of how SIGMA’s hip rehabilitation system works.

Dr McCormick explains the five key phases of post-hip procedure rehab, how we align the therapy team, surgeon follow-ups, and return-to-activity milestones. Whether you’re recovering from hip arthroscopy, tendon repair, or replacement, this video sets expectations and shows why our “100-Day Recovery” path is more than just a protocol—it’s a full collaboration.

Why Our Hip Protocols Stand Apart

Aviation Precision • Measurable Outcomes • Faster, Safer Return to Function

Criteria-based progression.

Criteria-based progression. Not just timelines — objective milestones to advance safely.

Reduced complication risk.

Stepwise load progression minimizes stress on healing tissues.

Return-to-performance mindset.

Strength, stability, and movement mechanics built into every phase.

Designed for both patients and clinicians.

Easy-to-follow instructions, clear precautions, and structured milestones.

If you are a therapist treating a SIGMA patient, please download the appropriate protocol and contact us anytime for clarification.

HIP REHABILITATION PROTOCOL LIBRARY

Below you’ll find surgeon-developed protocols for the most common hip procedures. Each includes phases, goals, precautions, red-flag signs, and return-to-activity benchmarks.
Hip Arthroscopy – Labral Repair

A structured, phase-based program emphasizing early protection, controlled mobility, progressive strengthening, and sport-specific reconditioning.

Includes:

    • ROM progression
    • Core & pelvic stabilization
    • Return-to-running criteria
    • Manual therapy guidelines
Hip Arthroscopy – FAI

Designed to optimize mobility restoration while preventing early overload of the capsule and labrum.

Includes:

  • Early motion restrictions
  • Safe strengthening timelines
  • Pelvic/hip stabilization
  • Return-to-sport criteria
Gluteus Medius / Minimus Tendon Repair

A high-precision protocol for abductor repairs, focusing on protecting the repair, preventing Trendelenburg gait, and prioritizing progressive neuromuscular control.

Includes:

    • Protected weightbearing guidelines
    • Abductor strengthening progression
    • Gait retraining
    • Sport-function reintegration
Proximal Hamstring Tendon Repair

Progressive program balancing graft protection with early functional gains, designed for athletes and high-demand patients.

Includes:

  • Hip flexion precautions
  • Hamstring loading progressions
  • Running & sprinting return program
  • Plyometric guidelines

Surgery for trochanteric bursitis is not common. When it is needed, we apply the same checklist-driven, minimally invasive philosophy used in our hip arthroscopy and abductor repair programs.

How to Use These Protocols

For Patients

  • Print or save the protocol for your surgery.
  • Bring it to every physical therapy visit.
  • Review the weekly goals and milestones so you know exactly what to expect.
  • Contact our office for any pain concerns, setbacks, or unusual symptoms.

For Physical Therapists

  • Follow the criteria-based phase progression.
  • Reference the strengthening and load guidelines.
  • Use the red-flag list to identify complications early.
  • Reach out to our team with any questions — continuity of care is vital.

For Athletic Trainers & Strength Coaches

  • Use return-to-sport criteria to guide load progression.
  • Incorporate hip and pelvic stabilization work before sport-specific drills.
  • Communicate regularly with the patient and therapist to ensure safe progression.

This approach ensures every patient — from elite athlete to active adult — follows a defined mission profile from injury to full recovery.

What Patients & Therapists Ask Most

Most programs begin within the first week after surgery, unless otherwise specified.
This depends on leg dominance, pain control, narcotic use, and surgeon clearance — typically after discontinuing crutches and regaining safe reaction time.
Soreness and stiffness are expected. Sharp pain, intense swelling, or mechanical catching should prompt a call to your surgeon.
Any physical therapist, athletic trainer, or performance specialist working with a SIGMA patient.
